This webpage provides links to companies that administer medical, dental, vision, prescription drug, mental health/substance abuse and life insurance benefits under the nationally negotiated benefit plans. It also includes toll-free telephone numbers and claim mailing addresses.
In addition, you will find information about features offered on the providers’ websites to assist you in obtaining “on-line” eligibility, claim and benefit status, as well as general health and wellness information.
Also, you will find current and archived new stories about the health care plans for reference.
Some of the items found below are in Adobe’s Portable Document Format (.pdf). It is necessary to have the free Adobe Acrobat Reader to open and view these items. If you do not have the Adobe Acrobat Reader installed on your computer, you can obtain it free of charge at www.adobe.com.

To view a list of health benefit providers along with links to their respective websites, click on the link below. To obtain a document in PDF format containing the telephone numbers and mailing addresses of the benefit providers, click on the second link.
If you are not working due to disability, you are required to keep timely “Proof of Disability” on file with Railroad Enrollment Services for uninterrupted benefits. The “Proof of Disability” form can be printed so your doctor can complete and fax to you health benefit provider.
